EFU Life’s COVID-19 Care benefit - Provisions and Conditions:

• COVID-19 Care is a complimentary cover for all existing and new policyholders/participants who hold In-force unit-linked
  policy/policies during the Coverage Period. This benefit is in addition to the contractual benefits of the original In-force policy/policies.
• Subject to the terms set out herein, COVID-19 Care shall provide coverage upon Death of the life assured/ participant, as per the
  following details:

• COVID-19 Care benefit would be payable in lump sum only once for the Covered Event, regardless of the number of policies issued by
  EFU Life covering the same life assured. The amount of COVID-19 Care benefit is capped at Rs 100,000/- per life assured/ participant.
• No amount shall be payable under COVID-19 Care for policies where, (i) the life assured/participant is diagnosed with COVID-19
  within 14 days from the Policy Issuance Date or the Policy Reinstatement Date; or (ii) the life assured/participant shows signs and/or
  symptoms of any respiratory illnesses or diseases before applying for the policy, i.e. before the Proposal Signing Date.
• The pay out to the beneficiary under COVID-19 Care will be subject to the policyholder/participant being infected with COVID-19, as
  well as diagnosed by a Registered Medical Practitioner during the Covered Period in Pakistan.
• The beneficiary/nominee of the Policy shall be responsible for submitting all relevant supporting documents to EFU Life.
• Notwithstanding anything herein to the contrary, any amount paid out to the beneficiary/nominee under COVID-19 Care shall be at the sole
  discretion of EFU Life.
• Definitions:
  "COVID-19” means confirmed cases of “Novel Coronavirus Disease 2019", as defined by the World Health Organization and confirmed
  by relevant medical reports.
  “Registered Medical Practitioner” means a medical practitioner registered and licensed by the Pakistan Medical and Dental Council, who
  is qualified in western medicine and is authorized to provide medical services where the life assured/participant was diagnosed with
  COVID-19 infection. The Registered Medical Practitioner who made the diagnosis must not be the Policyholder, life assured/participant or
  the family member, employer, employee, business partner of the Policyholder, or otherwise be affiliated (whether directly or indirectly) to
  the Policyholder or life assured/participant.
• EFU Life reserves the right to change the Provisions and Conditions, withdraw or cease all or any part of the COVID-19 Care at any
  time, without prior notice.
• COVID-19 Care does not affect or change any Provisions and Conditions of the original policy contract presently in-force with EFU Life

What is COVID-19 Care?

• COVID-19 Care is a complimentary cover for all existing and new policyholders/participants who hold in-force unit-linked
  policy/policies during the Coverage Period. This benefit is in addition to the contractual benefits of the original In-force
  policy/policies.

Who is eligible for COVID-19 Care?

• Both existing and new policyholders/participants are eligible to receive coverage under COVID-19 Care benefit. For existing
  policyholders/ participants cover is applicable if any of their policy/ policies are In-force.

When will coverage under COVID-19 Care start and when will it end?

• The Covered Period for this benefit will start from 23rd March 2020 and will end on 30th June 2020.

What are the qualifying criteria for COVID-19 Care?

• The policyholder/participant will receive COVID-19 Care benefit if the life assured/participant is diagnosed with COVID-19 during
  the Covered Period, subject to the following conditions:
  A) The life assured/participant is diagnosed with COVID-19 by a Registered Medical Practitioner.
  B) The policy/ policies must be In-force at the time of diagnosis.
  C) For policies issued/Reinstated between 23rd March 2020 till 30th June 2020, the life assured/participant must not be diagnosed
  with COVID-19 within 14 days after the Policy Issuance Date or the Policy Reinstatement Date (whichever is later) and the life
  assured/participant must not have shown signs and/or symptoms of any respiratory illnesses or diseases before the application
  date, i.e. Proposal Signing Date.

What documents must be submitted to claim COVID-19 Care benefit?

• We will require proof of diagnosis or Medical Certificate issued by a Registered Medical Practitioner.
  If the life assured/participant is diagnosed with COVID-19, then the following documents would be needed to make a claim under COVID-
  19 Care:
  A) COVID-19 Care Benefit Claim Form, available at our website: https://www.efulife.com
  B) Copy of the life assured’s/participant’s identification document.
  C) Medical Certificate issued by a Registered Medical Practitioner certifying the diagnosis of COVID-19; and if the life
  assured/participant was hospitalized, an in-patient summary certifying that hospitalization was due to COVID-19.

Must the medical documents be issued by a Registered Medical Practitioner in Pakistan? If the life assured/participant is
overseas and diagnosed with COVID-19, would EFU Life recognize medical
certifications issued by an overseas medical practitioner?

• We will accept medical documents issued by a Pakistani Registered Medical Practitioner, who is qualified, legally licensed and
  authorized to provide medical services by the Pakistan Medical and Dental Council. Further, life assured/participant must be
  diagnosed with or treated for COVID-19 in Pakistan.

Would I be notified of the claim status? How is the payout made?

• We will inform the beneficiary/ nominee in writing on the claim status and the cheque payment, if any, will be attached with our
  letter.

If a policy is applied for on 23 March 2020 but it has yet to be issued when the life assured/participant is diagnosed with
COVID-19, will he/she receive the lump sum payment?

• COVID-19 Care is only offered under an in-force policy which has been issued or has taken effect (whichever is the later) 14 days
  before the date of diagnosis. Further, the life assured/participant must not have shown any signs and/or symptoms of any
  respiratory illnesses or diseases before applying for the policy. So, the policy with Issuance Date of 23 March 2020 would be
  eligible if the date of diagnosis is 14 days after the Issuance Date. Pending proposals are not eligible for any benefit.

Will the payout upon death under COVID-19 Care affect the death benefit under existing policy(ies)?

• The COVID-19 Care benefit is in addition to the existing coverage available under any In-force policy(ies) with us.

Do I need to pay additional premium for this coverage?

• As this is a complimentary financial protection against COVID-19 by EFU Life, no additional payment needs to be made.

  The amount of COVID-19 Care benefit is capped at PKR 100,000 per life assured/participant. This means that COVID-19 Care
  benefit amount of Rs 100,000, for the Covered Event, will only be paid out once, regardless of the number of In-force policies
  covering the same life assured/participant.

To submit a claim under COVID-19 Care, are incurred expenses such as the medical report fee, copies of test results etc.
reimbursable by EFU Life?

• All expenses incurred shall be borne by the beneficiary/ nominee of the policy.

Can I purchase COVID-19 Care as a standalone plan?

• No, COVID-19 Care benefit is extended to our eligible customers on a complimentary basis. Customers cannot purchase this
  benefit as a standalone plan.
